HEALING VILLAGE

PARTICIPANT CONSENT & ACKNOWLEDGEMENT

For Braingame, coaching, energy healing, circles, retreats and other Healing Village services

This is the piece I would actually put into your registration/checkout process. Because sensitive-information consent should be adequately informed, voluntary, current and specific, I would use an unticked checkbox requiring affirmative action, rather than hiding this inside general Terms.

 

By registering, I acknowledge and agree that:

I understand the nature of the service. I understand that Healing Village offers personal-development and empowerment coaching, spiritual education and complementary practices that may include Reiki, energy healing, meditation, circles and other spiritual or reflective practices.

I understand that these services are complementary. They do not replace appropriate medical, psychological or psychiatric diagnosis, treatment or care. I remain responsible for obtaining professional healthcare or other professional advice when appropriate and will not discontinue prescribed treatment or medication solely because of Healing Village teachings or services.

I accept responsibility for my participation. I understand that Healing Village can provide teachings, tools, guidance, healing practices, support and a container for my journey, but my choices, participation, actions and wellbeing remain my responsibility.

I understand that results cannot be guaranteed. Personal, spiritual and transformational experiences differ between individuals, and no particular healing, health, emotional, relationship, financial, professional or life outcome has been promised to me.

I choose to participate voluntarily. I understand that I may be invited to explore practices such as meditation, reflection, journalling, intention-setting and aligned action, and I remain responsible for deciding what is appropriate for me.

I understand my financial commitment. I have considered my decision before enrolling. I understand that Healing Village does not ordinarily provide refunds merely because I change my mind, do not participate or complete the program, or do not achieve the result I hoped for. Where I select an instalment plan for a fixed-price program, I understand that it is ordinarily a payment method for the agreed total program price rather than a pay-as-you-go arrangement. My rights under Australian Consumer Law remain unaffected.

I will respect intellectual property. I understand that Healing Village and Braingame teachings, materials, frameworks, videos, workbooks and other proprietary content are supplied for my personal use and may not be copied, distributed, taught, resold or commercially exploited without written permission.

I will respect other participants' privacy. I will not share, screenshot, record or publish another participant's personal information or story without their permission. I understand that Healing Village asks all participants to maintain confidentiality but cannot guarantee the behaviour of independent group members.

I consent to relevant information handling. Where I voluntarily provide health or other sensitive information reasonably necessary for the Healing Village service I have requested, I consent to Healing Village collecting and handling that information for the purpose of providing and administering that service in accordance with the Healing Village Privacy Policy. I understand that I can contact Healing Village if I have questions about this consent or the handling of my information.

For an intake form where you're actually asking about health/wellbeing information, I would add a separate checkbox:
